About this event

The symposium included sessions on value-based healthcare at a system level and the move to more integrated care for local populations. The day included speakers from overseas as well as the UK and encouraged interactive discussion to tackle the key challenges as well as sharing case studies and success stories.

    Europe's goal is to provide a platform to share information on value-based health care (VBHC) implementation. They also host the international VBHC prize competition each year which recognises projects that have adopted a fundamentally new line of thinking in creating value for patients in terms of real outcomes, real cost reduction, real connections, one common language and are doctor-led initiatives. Prof. Dr. van Eenennaam shares his insights on the practical implementation of value-based healthcare theory and how organsations and systems can maximise value by centering care around the patient.

    H2O is a strategic partnership across multiple countries. The project aims to create a robust data governance and infrastructure model to collect and incorporate patient outcomes at scale into healthcare decision making at an individual and population level. In this session the presenters will provide an overview of the project as well as providing a more detailed look at the national H2O observatory in Spain and the impact the project will have on improving outcomes for patients.

    The North Karelia Social and Healthcare Joint Municipal Authority (Siun sote) has organised and delivered the public social and health care in North Karelia, eastern Finland, since the beginning of 2017. They have developed health and social care packages with the person at the centre and have calculated for the first time in Finnish economic history the real costs of integrated social and health care services. Siun sote continues to improve the care pathways of patients with multiple and complex needs whilst simultaneously working to understand the ‘big picture’ and the whole system view of integrated services. In this session Dr Kivinen will give an overview of the Finnish approach and discuss how they have embraced system working and the impact it has had on patient care and costs.

    In this session delegates will be guided through a real-life international case study example focusing on maternity services in Kenya. The session will explore value-based healthcare in practice with the opportunity for attendees to input at key decision points through a simulation exercise. Delegates will explore the practicalities of measuring outcomes in obstetric care, navigating how to improve access to and experience of care by pregnant women.

    What happens if rather than looking at individual patient pathways, health systems look at patients and service users more holistically? At Mersey Care NHS FT, the trust has taken the approach of segmenting its population into groups with common healthcare needs and risk-stratifying these groups. This view has led to the launch of System P which looks at a preventative, predictive, precise approach to population, patient, and person in a joined-up intelligence-led system. In this session Wes will give an overview of the project and discuss how they are working to ensure resources are invested wisely within the health system to optimise the outcomes for the population. This approach will be most effective when underpinned by system-wide cost and activity data so Wes will also share details of work underway to join up and use data across the system to inform decision making.

    Oak Street Health is a network of primary care centres for adults on Medicare in the US and is recognised as a world leader in putting the theory of value-based care into practice. The Oak Street approach focuses on quality of care and outcomes for patients rather than volume of services. As a result, the organisation delivers measurably better outcomes and drives cost savings. Many of the communities that Oak Street serves have had limited access to routine primary care; by investing in community programs to encourage patients to engage with primary and preventative care, they have been able to start to address the health disparities facing these communities. In this session, Dr. Khan will give an overview of the Oak Street Health approach and share details of the ways they have used value to work towards health equity.
