The Integrated Care Centre serves Hull and the East Riding of Yorkshire. The clinical leaders have worked collaboratively with the CCG and health and social care providers to deliver a new and award-winning anticipatory and responsive community frailty model.
This has resulted in a sustained reduction in unnecessary emergency care and primary care attendances and inappropriate polypharmacy costs. The team have also implemented an urgent frailty advice and guidance line which has been essential for the delivery of safe community-based care during the Covid-19 pandemic.
