The NHS long-term plan for England, published in January 2019, promises to focus on population health which will be enabled by Integrated Care Systems covering the whole country by April 2021. In this webinar, you will hear how Cwm Taf Morgannwg University Health Board in South Wales are harnessing the benefits of integration by using data to demonstrate how investment in primary care can reduce pressure on secondary care beds  – and use this information to deliver value. 

Kendal will explain how the Health Board have started to  make use of rich secondary care datasets to profile the population and create a system that can measure the impact of improved condition management in the community on secondary care bed pressures. You will hear how the findings of this project have provided evidence to expand services in the community, in order to realise long-term efficiencies and improve patient care.
