    Having read the articles on the implementation of patient-level costing and service line management at Chelsea and Westminster FT, what are the obstacles you are facing with your own implementation? Or do you have any top tips for ensuring the successful introduction of patient-level costing?

    The concept of SLR/PLC is an extension of several other ideas that have fizzled out in the course of my practice. Tied to SLR is idea that you get paid for what you do and that, if you are a successful unit that makes any savings (profit against tariff) in the delivery of the service, you get to keep it and invest in current and future development, including innovation; very convincing and motivating as a concept. However, this has never happened in my experience and I wait to see the day where success, rather than failure, is rewarded, rather than punished. I also think that SLR is an averaging exercise which, by the backdoor, is another CRES exercise that results in communal (pan-NHS) contraction of remuneration per activity. This would not be unreasonable considering the current financial constraints but in the long run strangles the process, introduction and implementation of innovation. I do not (also) think (based on my experience) that the NHS can either develop, or procure, a robust well thought out information management system that will deliver pan -NHS BPR based on SLR/PLC that will be fit for purpose. But if it does come close, the learning cycle and benefits accrued will be years, if not decades, down the line. ??keh??

    We are looking at putting processes and systems into our Trust to support SLR and ultimately Patient Level Costing. I am looking for some contacts throughout the UK that have already implemented these within their organisation, particularly for specialist services such as Cancer, in the hope that we could learn from their experiences and consider what system options we have.
