Nomination for the 2009 Awards
To nominate an individual, team or organisation for one of this years HFMA Awards, please download a copy of the entry template. This should be completed and submitted electronically together with any supporting appendices to awards@hfma.org.uk
All complete entries must be received by Friday 2nd October 2009.

NB Please mark clearly the organisation name, team name or individual’s names you are entering, and the award you wish the application to be considered for. Please also supply the name of at least one contact point for each application. All entries will be acknowledged.
